    My egfr 40 last 35 day ago. Now increase to 59. I just controlled my protein intake, cut sugar and monitor salt intake. Everyday im skip my breakfast. Hope my egfr back to normal within 2 months

    Thanks Dr. Cronin! I’m a patient who follows your channel, and am re-testing as my LDL jumped to 160 in one year! Menopause I think, as my diet hasn’t changed. Good to know this information in case I need to go on a statin.

    Why would the development of Calcium oxalate crystals due to ethylene glycol toxicity damage the kidneys since they only form kidney stones and these can be easily removed.

      The calcium oxalate crystals form in the kidney tissue itself causing damage to the cells. Kidney stones form in the urinary collecting system.

    How often/in what situation do you check calcitonin?

      I've never checked a calcitonin level. This blood test is used for the evaluation for medullary thyroid carcinoma, not for the evaluation of hypercalcemia.

      @@BCNephro Do you always see D 1,25 low in subclinical hypercalcemia where blood calcium isn't yet out of range? I've been talking to a few people that have falling eGFR and low PTH but claim to have normal D levels.

      A low 1,25 vitamin D level would be expected in CKD with secondary hyperparathyroidism, with the elevated PTH maintaining the calcium in the normal range. However I don't typically check 1,25 vitamin D in this situation. An elevated 1,25 vitamin D can be a cause of hypercalcemia in granulomatous diseases like sarcoidosis or lymphoma. This would be a part of my evaluation in a patient with hypercalcemia and a suppressed PTH.

      @@BCNephro A lot of tofu, chickpeas, and kidney & black beans. I do feel tired often, but also have insomnia I believe due to menopause. Stage 3A with 1+ protein.

    Is this drug recommended for reducing proteinuria in non-diabetic patients with normal GFR?

      Yes guidelines recommend it for this indication. Although I typical reserve it for patients with overt albuminuria already on a renin angiotensin system blocker.     Thanks for the video. Its very hard to stop PPIs considering the great relief they bring for GERD. Without this medication I would almost certainly have Barrett's or peptic ulcer. I have a gastric hiatal hernia which is probably the main problem. My plasma creatinine has risen too over the last 10 years or so possibly due to PPIs but possibly also to uncontrolled hypertension I had for a short time. Esomeprazole seems the least renal damaging. I also substitute this with a H2 blocker (famotidine) three times per week. Loss of renal function is a very real concern with PPIs. Reducing abdominal fat would help a lot but this is difficult.

    Ive been suffering from low urine output for almost 3 months, my crea,bun, urinalysis and kub are all normal. Im having headache sometimes and skin burning sensation along with pin and needles and feeling hot but no fever.. my nephro has no clue what is happening to me. Doc whats wrong with me?

    Excellent video with very useful and valuable info. . I had a multi big stone issue in left kidney only about 8 years ago that has made my left kidney atrophic by about 1cm smaller. Function was about 15% last I checked 6 years ago . How often do you suggest I do a renal comparison scan for function? What determines removal function or pain? Surgery was not able to remove all stones because some 10mm ones are stuck in lower pole calyx. My GFR is in the 60 -65 area and I have heard removing a kidney even with limited function drops GFR significantly . Thanks ,,Love the channel ..

      This is not medical advice, check with your doctor, but I can share my experiences. I typically see atrophic kidneys removed for chronic infection. If I want to predict a post nephrectomy GFR I multiply the pre nephrectomy GFR by the % function of the remaining kidney (for example 60-65 x 85% = about 51-55). When there is an atrophic kidney I usually monitor by ultrasound (mainly to monitor how the good kidney is doing), but I would repeat the scan if surgery was planned.

      Hi, in most cases the 2021 CKD-EPI creatinine based equation would be adequate. If there is an extreme in muscle mass would use the cystatin c equation and/or 24 hour urine creatinine clearance. If drug levels are available that should be done.

    I am 44 years male , Indian Origin living in Australia, Regular Gym goer , My Creatinine is 1.74 mg/DL ( EGFR = 47 ml/min/1.73m2) based on which I was told Stage 3 CKD. I go to gym 6 days a week and eat high protein diet so I almost forced my Doctor to do a Cystatine C test EGFR and guess what my Cystatine C is 0.77 mg/L ( EGFR 113 ml/min/1.73m2) . Doctor was really angry ....Go figure

    Here’s my approach to resistant hypertension: 1. ARB Telmisartan is my favorite 2. Nifedipine (may use as first line if African American or pulmonary hypertension) 3. Chlorthalidone Once you’re maxed out on 3 agents screen for causes of resistant hypertension: pheochromocytoma, hyperaldosteronism, Cushings, hyperthyroid, renal artery stenosis, and of course med non adherence, obesity, and excessive sodium intake 4. Spironolactone 5. Carvedilol 6. Minoxidil 7. Hydralazine I don’t prescribed clonidine; the rebound effects are a nightmare. In my experience every patient can be controlled with lifestyle, the above medications, and management of comorbidities

      Thanks for outlining your approach. I agree with much of it specifically using an ARB first, that extended release nifedipine is more potent than amlodipine, chlorthalidone is more potent than HCTZ and that spironolactone is 4th agent over carvedilol. By the time patients get to me they are often on clonidine and I am proactive in attempting to wean.

      You guys know that eGFR and creatinine results are often false.... check CYSTATINE C.... I have 55 eGFR (which is false) while the cystatine c blood result shows that I have 100% functioning kidney....
